I wonder why they didn't simply dub the original Spanish movie - virtually all foreign movies are dubbed with German voices here, and if the voice actors do their job, it's as good as the original voices. Hey, it's just natural. 
Considering the fact that his species exchanges genetic material with their hands, I assume that, since he's clearly amphibious, it's some kind of swim bladder, or maybe even his lungs.
